全胰腺切除术后患者血糖特点、并发症及治疗的研究进展

【摘 要】对于胰腺恶性肿瘤或弥漫性病变、重度慢性胰腺炎患者，目前临床主要采用全胰腺切除术进行治疗，此类人群术后出现胰源性糖尿病，血糖波动大，易出现反复低血糖。术后并发症与血糖水平密切相关，因而良好的血糖控制对于提高患者生存率至关重要。既往对全胰腺切除术后患者的血糖情况，我国仅有少数或个案报道。本文主要阐述全胰腺切除术后患者的血糖特点、相关并发症及治疗的研究进展。%Total pancreatectomy has been mainly used to treat malignant diseases of the pancreas, di-ffuse ductal involvement, and severe chronic pancreatitis. Patients may develop pancreatic diabetes after the sur-gery, in whom the glucose levels fluctuate widely and are hard to control, which increases the incidence of recur-rent hypoglycemia. Postoperative complications are closely associated with the glucose levels. Thus, tighter glyce-mic management is essential for increasing the survival of these patients. Few cases have been discribed in China. In this article, we reviewed the blood glucose features, peri-and post-operative complications, and clinical treat-ment for patients undergoing total pancreatectomy.
